Your First Visit

Downloadable Forms

You’re coming to an office because you want to see a dentist; not be handed a clipboard. Make it easy and download all our patient information forms now. Simply complete the forms and bring them to your first visit. See? Things are already easier.

What Happens at my First Visit?

You will get a careful oral examination by your dentist, who will check for things like tooth decay, gum disease and other things that can affect not just your mouth, but your overall health. Then you’ll talk together about any treatments you might benefit from.

You may get a teeth cleaning on this visit, or it may be scheduled for a follow-up visit. From start to finish, this first visit will most likely last 1-1 1/2 hours so we can make sure we’ve given your smile the attention it needs.

What to Bring With You

Let’s make sure your visit is a breeze. Don’t forget to bring:

  • Dental X-rays from the last six months.
  • We can take X-rays if you don’t have them.
  • Your medical and dental history info, including any medications you’re taking
  • Your dental insurance card (if you have insurance)
  • Your parent if you’re under 18 (of if you’re over 18, but just want the company)

Want to get a head start on the forms ahead of time? You can download them to the left of this page and print and complete them at home.

New Patient Exam

When you arrive for your new patient exam, you will be greeted by our friendly front office staff who will help you fill out any paperwork and get you seen promptly by the doctor.

Our new patient exam includes the following:

Digital X-Rays

Your exam includes full-mouth digital X-rays. Digital X-rays emit 80% less radiation than traditional film X-rays, and the images can be produced much faster and with greater accuracy.

Intraoral Photos

An intraoral camera will be used to take digital photos of the inside of your mouth, and you will be shown the images on a screen during your exam.

Full Oral Exam and Consultation

The doctor will meet with you to conduct a full oral exam and find out your goals and needs. In the oral exam, your gums, teeth and jaw will be checked thoroughly so that any and all issues will be properly identified. The exam includes an oral cancer screening and periodontal exam, during which the doctor probes your gums and checks for signs of gum disease.

Diagnosis and Written Treatment Plan

The doctor will go over the results of your exam with you, taking care to clearly explain everything, answer all your questions, and gain an understanding of your particular goals. If you’re interested in dental implants or cosmetic dentistry, for example, the doctor will discuss these procedures in depth with you. All of your options will be presented, and you will get a treatment plan that best works for you and your budget.
